Job summary

Serenity Hospice Care Pennsylvania seeks a Registered Nurse (RN) Case Manager to join our compassionate hospice team in the Harrisburg area, delivering patient-centered end-of-life care and coordinating with the interdisciplinary team.

You will support families, maintain accurate documentation, uphold our commitment to dignity and quality care in a fast-paced environment. PA licensure required.

Qualifications

• Current Pennsylvania RN license or license-eligible in PA

Responsibilities

• Perform initial and ongoing patient/family assessments to develop care plans

• Ensure seamless care transitions and effective communication among team members
• Maintain accurate documentation of patient care and communications

Key Responsibilities:

• Perform initial and ongoing assessments of patients and their families to develop individualized plans of care
• Coordinates care with the interdisciplinary team, including physicians, social workers, and other healthcare professionals
• Provides ongoing patient and family education and support to ensure optimal care and quality of life
• Identifies and addresses potential barriers to care, such as psychosocial and spiritual concerns, and develops strategies to overcome them
• Collaborates with the hospice team to ensure seamless care transitions and effective communication with all involved parties
• Maintains accurate and timely documentation of patient care and communication with the healthcare team

Requirements:

• Current licensure as a Registered Nurse (RN) in the state of Pennsylvania
• Strong clinical knowledge and skills, with a focus on hospice and palliative care
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
• Strong organizational and prioritization skills, with the ability to manage multiple responsibilities and deadlines
• Ability to work in a fast-paced environment and adapt to changing situations and priorities
