Job summary

PruittHealth Hospice is seeking a compassionate PRN Nurse Practitioner to join our hospice and palliative care team serving patients in the home or facility setting. You will conduct comprehensive adult/geriatric assessments, develop plans of care, and collaborate with an interdisciplinary team to deliver compassionate, patient-centered care.

Qualifications

• Current, active, and unrestricted RN and NP licensure in state of practice.
• Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.

Responsibilities

• Proactive, collaborative team member in a hospice care environment
• Conducts comprehensive adult/geriatric assessments of hospice patients and evaluates and manages plan of care
• Commitment to caring for patients and partners
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
