Valor Hospice in Fort Worth, TX is seeking a Part Time Registered Nurse to administer in-home nursing care for patients of all ages, coordinating with the Interdisciplinary Group, families, and referring agencies. The role emphasizes comprehensive assessments, plan-of-care development, and education on end-of-life care.

The RN must hold an active license, have at least 2 years of clinical RN experience (hospice preferred), and possess reliable transportation.

Qualifications

Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state RN license and a valid driver’s license.
Minimum 2 years RN experience in a clinical setting; hospice experience preferred.
Good written and spoken English.

Responsibilities

Initial and ongoing comprehensive nursing assessments of patients and families.
Develop, implement, and document the plan of care with the IDG/Team.
Coordinate care with patients, families, and referring agencies.
Educate patients and families about disease process, end-of-life care, nutrition, and self-care.
Direct and guide Hospice Aide and LPN/LVN to ensure quality care.
Document progress notes and communicate changes to providers.
Monitor cases for third-party payor requirements and ensure compliance.
