Job summary

Constellation Health Services seeks a Nurse Practitioner to provide comprehensive hospice services in patients’ homes and communities under physician supervision. You will evaluate health history, diagnose conditions, manage plan of care, and coordinate with the hospice interdisciplinary team.

Responsibilities include completing face-to-face visits, ensuring eligibility, and guiding families through end-of-life care while maintaining HIPAA and regulatory compliance.

Qualifications

• Graduation from an accredited School of Nursing.
• Accredited Nurse Practitioner Program.
• Current DEA license.
• Current nursing licensure in state and CPR certification.

Responsibilities

• Provide medical care and manage terminal illness under supervision of collaborating physician.
• Complete history and physical examinations, order/interpret tests and document in medical record.
• Develop plan of care, prescribe medications and coordinate hospice services.
• Report hospice eligibility and recertification to appropriate medical director per regulations.
• Educate patients and families, collaborate with interprofessional team, ensure continuity of care.
