Job summary

Adoration Health is seeking a Hospice Nurse Practitioner to deliver compassionate hospice care, evaluate and manage terminally ill patients, and collaborate with an interdisciplinary team. You will develop plans of care, prescribe appropriate medications and treatments, and ensure compliant documentation and regulatory adherence.

The role requires NP certification with state licensure, CPR, and ideally hospice/palliative care experience.

Qualifications

• Graduate from an accredited School of Nursing and Nurse Practitioner program.
• Master's degree with minimum one year NP experience.
• Current nursing license in state of practice.
• Current CPR certification.
• DEA license if applicable to state regulations.
• Accreditation from an approved certifying body for advanced practice nursing as required by state.
• Certification in Hospice/Palliative care preferred (CHPN) or Geriatrics.
• One year hospice or palliative care experience preferred.

Responsibilities

• Provide hospice medical care, palliate and manage terminal illness under supervision of a collaborating physician.
• Complete history and physical examinations evaluating patient condition and health history.
• Meet hospice face-to-face visit requirements to assess eligibility for each period.
• Recommend certification/recertification decisions to hospice medical director.
• Maintain accurate clinical notes and documentation.
• Communicate with physicians and participate in interdisciplinary team meetings.
• Ensure compliance with state and federal regulatory requirements.
• Perform additional duties as assigned.
