Adoration Health Hospice Nurse Practitioners provide medical care, palliation, and management of terminal illness in accordance with a supervising physician and state Nurse Practice Act.

Responsibilities include evaluating health histories, completing hospice visits, determining eligibility for certification/recertification, maintaining clinical notes, and collaborating with physicians and care teams to support patients and families.

Qualifikationen

• Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
• Current nursing license in state of practice.
• Master’s degree with minimum one year NP experience.
• Current CPR certification.
• One year hospice or palliative care experience preferred.
• Current DEA license if applicable to state regulations.
• Certification in Hospice and Palliative Nursing (CHPN) or Geriatrics preferred.
• Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.

Aufgaben

• Providing medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
• Completing history and physical examinations by evaluating the patient's medical condition and health history.
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility.
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
• Completing and maintaining accurate clinical notes.
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers.
• Ensuring compliance with all state and federal legal and regulatory requirements.
• Additional responsibilities as assigned.
