Zusammenfassung
Symbii Home Health in Utah seeks a full-time Nurse Practitioner to provide comprehensive hospice services under physician supervision in the Davis and Weber County regions. The NP will evaluate health histories, develop plans of care, prescribe treatments, and coordinate with patients, families, and healthcare teams. The role requires a Master's in Nursing, NP certification, state licensure, and CPR.
Experience in hospice/palliative care is preferred. Travel to patients’ homes is expected.
Qualifikationen
- Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
- Master's degree with at least 1 year NP experience and hospice/palliative care experience.
- Current nursing licensure in state and CPR certification.
- Accreditation from an approved certifying body for advanced practice nursing as required by State.
Aufgaben
- Provide medical care and manage terminal illness under physician supervision.
- Complete history and physical examinations and interpret test results.
- Develop individualized plans of care, prescribe medications, and coordinate hospice care services.
- Assess hospice eligibility and complete required visits and documentation.
- Communicate with the hospice medical director regarding eligibility and care plans.
- Coordinate and oversee patient care with the interdisciplinary team.
- Educate patients and families on care, prevention and end-of-life issues.
- Maintain clinical notes and documentation in the medical record.
- Ensure safety and continuity of care for patients and families.
- Establish relationships with staff and community referral sources to support program growth.
