Christus VNA Hospice, part of LHC Group and Optum family, seeks a Hospice Nurse Practitioner to provide palliative and supportive care, coordinating with physicians and an interdisciplinary group.

The role focuses on consultations, plan-of-care initiation, and patient/family education, with a commitment to compassionate, patient-centered care and collaboration with the Medical Director.

Qualificações

• Active unrestricted RN license in the state of practice.
• Current certification as a Nurse Practitioner or Clinical Nurse Specialist.
• National board certification such as ANCC, NBCHPN, or AANP.
• At least 5 years of nursing experience.
• Valid driver's license and reliable transportation.

Responsabilidades

• Make face-to-face Hospice visits within prescribed time limits and report findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director as needed, inform the primary physician of recommended services, and collaborate with other physicians as needed.
• Initiate and coordinate the plan of care. Document problems, goals, interventions, and patient/family response; collaborate with patient/family, attending physician and IDG.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and report patient condition and needs to Administrator and Hospice Medical Director.

Nurse Practitioner / CNS certification
