Job summary

AccentCare, Inc. is hiring a Be the Best Hospice After Hours Admission RN to join a dedicated team in New Castle County, Delaware. The role involves on‑call admissions, nursing assessments, and coordinated care planning.

The ideal candidate holds an RN license for the applicable state, with hospice/home health experience preferred, and is prepared for after‑hours weekend shifts. A supportive benefits program accompanies the role.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required; associate degree preferred.
• CPR certification recommended (required for direct patient care where applicable).
• Prior hospice/home health, medical-surgical, or geriatrics/pediatrics experience is valued.

Responsibilities

• Conduct comprehensive nursing assessments and collaborate with patients, families, physicians and the IDG to develop and evaluate care plans.
• Maintain timely, accurate documentation and ensure plans reflect patient status.
• Provide education and emotional support to patients and caregivers, participate in on‑call rotations and admissions.
• Coordinate all aspects of patient care if serving as Care Manager, including scheduling, medication management and supervision of staff.
• Respond to triage calls and support care transitions in on‑call capacity; participate in IDG meetings as directed.
