AccentCare in Florida is seeking a dedicated RN for after-hours admissions in a full-time role. You will perform comprehensive nursing assessments, coordinate patient care with the interdisciplinary group, and provide education and emotional support to patients and families.

The position requires an active RN license in the state, willingness to participate in on-call rotations, reliable transportation, and strong communication skills.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required, with an associate degree preferred.
• Must demonstrate a willingness to participate in an on-call rotation and maintain reliable transportation and driver’s license.
• Strong communication and organizational skills are essential, with proficiency in EMR systems.

Responsibilities

• conducts comprehensive nursing assessments and collaborates with patients and IDG to develop and evaluate care plans.
• Maintains timely documentation and ensures care plans reflect current patient status.
• Provides education and emotional support to patients and caregivers and participates in on-call rotations, admissions and care transitions as directed.
• Coordinates all aspects of patient care including scheduling, medication management, plan of care oversight and supervision of staff when applicable.
• Responds to triage calls, performs visits and assessments, and participates in IDG meetings and family conferences.
