Job summary

AccentCare Inc. is seeking an After Hours Admission RN in Newark, DE for full-time night shifts (Friday, Saturday and Sunday 5:00pm–8:00am) within New Castle County.

The role requires an active RN license and prior hospice or related nursing experience, with the ability to participate in on-call rotations and handle physical patient care tasks. The position emphasizes care coordination, documentation, and collaboration with an interdisciplinary team, along with providing education and support to

Qualifications

• Active RN license in the applicable state; associate degree preferred.
• Experience in hospice, home health, medical-surgical, geriatrics or pediatrics.
• Willingness to participate in on-call rotation; valid driver’s license and state-required liability auto insurance.
• Strong communication and organizational skills; proficient in MS Office and EMR; ability to collaborate within an interdisciplinary team.
• Physical requirements: lifting patients up to 200 lbs with assistance; moving supplies up to 20 lbs for 500 ft; frequent standing, walking, sitting, stooping and reaching.

Responsibilities

• Conducts comprehensive nursing assessments with the IDG to develop, implement, and evaluate individualized plans of care aligned with regulatory requirements.
• Maintain timely and accurate documentation; ensure care plans reflect patient status and coordinate resources.
• Provide education and emotional support to patients and caregivers; participate in on-call rotations, admissions and care transitions.
• If Care Manager, coordinate all aspects of patient care including scheduling, medication management, plan oversight, IDG collaboration, and supervision of staff.
• If On-Call, respond to triage calls, perform visits and assessments, coordinate with the Administrator On Call; may attend IDG meetings.
