Job summary

AdvisaCare Hospice LLC is seeking a compassionate Medical Social Worker (MSW) to join our hospice team serving Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas in Michigan. The MSW will support patients and families, provide one-on-one compassionate care, and join a dedicated hospice team.

Qualifications

MSW degree from CSWE-accredited program.
One year of Home Health Care experience preferred.
Hospice experience preferred.
Valid Driver's License, car insurance and reliable transportation.

Responsibilities

Performs psychosocial, emotional, spiritual and bereavement assessments and helps develop goal-directed care plans.
Provides care under physician direction and participates in IDT conferences.
Documents all patient/family visits and referrals timely in patient records.
Conducts reassessments of patient/family needs and counseling as required.
Provides crisis intervention and individual or family counseling when indicated.
Acts as a care advocate for patient/family units and coordinates with community agencies.
