Job summary

AdvisaCare is seeking a dedicated hospice social worker to guide patients and families through the journey with compassion, advocacy, and thoughtful support. You will contribute social work expertise in a care setting where emotional, spiritual, and practical support are central to the patient and family experience.

You will conduct comprehensive psychosocial assessments, participate in interdisciplinary team conferences, and provide counseling and crisis intervention as needed.

Qualifications

• MSW degree from a CSWE-accredited program.
• Active LMSW or LLMSW license in Michigan.
• Prior experience working with patients and families.
• Valid driver’s license, car insurance, and reliable transportation.
• Eligible to work in the United States.
• Experience in home health care or hospice care preferred.

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments and help develop goal-directed care plans.
• Provide care under physician direction and participate in IDT conferences, sharing psychosocial, emotional, spiritual, physical, and financial information.
