Job summary

Graham Healthcare Group is seeking a Medical Social Worker (MSW) for Residential Home Health and Hospice in Arlington Heights, IL, serving surrounding areas. The role emphasizes autonomy, compassionate patient care, and collaboration with an interdisciplinary team.

You will conduct psychosocial assessments, develop care plans, provide counseling, and help families navigate end-of-life decisions while maintaining EMR documentation.

Qualifications

• Master’s in Social Work from graduate program accredited by the Council on Social Work Education.
• Current license as Medical Social Worker in the state of practice.
• 1–2 years post-graduate social work experience in a health care setting.
• Valid driver’s license and ability to travel ~20% of the day.

Responsibilities

• Deliver patient-centered care in a hospice setting as part of an interdisciplinary team.
• Conduct psychosocial and bereavement assessments and develop care plans.
• Provide counseling and connect patients/families with resources.
• Ensure accurate EMR documentation and advocate for patient wishes.
• Work independently while remaining connected to the team and travel to patient homes.

• Provide counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey with clarity and dignity
• Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation
• Work independently but never alone
• Travel to patient homes within assigned territory

MSW Qualifications

• Master’s in Social Work from graduate school accredited by the Council of Social Work
