John Paul VNA Hospice - a LiveWell Partners Network Agency in Michigan is seeking a dedicated Hospice Medical Social Worker (MSW) to support patients and families in end-of-life care. You will assess emotional, social, financial, and environmental needs and collaborate with the interdisciplinary team to develop care plans.

This full-time, in-the-field role involves home visits 15-30% of the time and mileage reimbursed at $0.55 per mile.

Qualifications

• Active Medical Social Work licensure in the state of Michigan.
• Master of Social Work (MSW) degree.
• 1 year of experience as a licensed social worker in a healthcare setting is required.
• 2+ years in an acute or post-acute care setting is preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments of hospice patients and families, identifying needs affecting quality of life and care plans.
• Develop and implement individualized social work interventions, providing counseling and support.
• Educate families about hospice philosophy, services, dying process, advance care planning, and resources.
• Connect patients with community resources, financial assistance, transportation, support groups, and social services.
• Collaborate with physicians, nurses, chaplains, aides, volunteers, and the interdisciplinary team.
• Provide bereavement support and referrals to grief resources for patients and families.
• Complete assessments, care plans, progress notes, and documentation in a timely, confidential manner.
• Conduct home visits and maintain professional relationships with patients, families, and referral sources.
• Participate in team meetings, quality improvement, and staff education.
