Job summary

moments Hospice seeks a compassionate Social Worker to deliver psychosocial services to terminally ill patients and families. You will coordinate with an interdisciplinary team, complete timely assessments, and help with bereavement and care planning while traveling for patient visits.

This role requires an MSW (preferred) or a BS in Social Work with healthcare experience, an active SW license, and strong resource knowledge to support patients, families, and caregivers in various settings.

Qualifications

• MSW degree preferred or BS with at least 1 year healthcare experience.
• Current Social Worker license required.
• Hospice experience is a plus.
• Knowledge of community resources and eligibility requirements.
• Strong organizational and communication skills.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Contact patient, family, and caregiver within 48 hours of admission.
• Perform Case Manager duties.
• Document psychosocial assessment within 24 hours of completion.
• Assess psychosocial needs of patient and family regularly.
• Support bereavement needs and pre-bereavement planning.
• Participate in weekly interdisciplinary team meetings.
• Assist with funeral arrangements if requested.
• Provide counseling during illness and at time of death if requested.
