Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice Medical Social Worker (MSW) to provide patient‑centered social work in patients’ homes in the Grand Rapids, MI area. You will assess psychosocial needs, connect families to resources, support care planning and advocate for goals that promote dignity and independence.

Qualifications

• Master's Degree from a CSWE-accredited social work program.
• Michigan MSW license required.
• At least 2 years of social work experience in health care.
• Ability to serve acutely and chronically ill patients.
• Ability to complete written, verbal, and observational tasks.

Responsibilities

• Provide compassionate, patient-centered social work in home care.
• Assess psychosocial needs and connect patients with resources.
• Support care planning and advocate for patient goals.
• Build meaningful 1:1 patient relationships.
