Job summary

AdvisaCare Home Health & Hospice in Michigan is seeking a Medical Social Worker (MSW) to support patients and families at home. You will perform psychosocial assessments, assist with goal-directed care, and collaborate with the care team to address emotional and spiritual needs.

This role offers flexible part-time or per diem opportunities across Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas.

Qualifications

• MSW degree from a CSWE-accredited program.
• One year of Home Health Care experience preferred.
• Hospice experience preferred.
• Valid driver''s license, car insurance, and reliable transportation.

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in developing and implementing goal-directed care plans.
• Supports care under physician supervision and participates in IDT conferences, addressing psychosocial, emotional, spiritual, physical and financial needs.
• Documents all patient/family visits and referrals in the patient record.
• Conducts reassessments of patient/family needs and counseling as required.
• Provides crisis intervention and individual or family counseling when indicated.
• Acts as care advocate for patient/family unit; maintains community referrals and interagency cooperation.
• Provides ongoing counseling related to death and dying as needed.
• Participates as a Bereavement Team member as assigned.
• Adheres to Hospice policies.
