Job summary

Trinity Health At Home is seeking a Hospice Medical Social Worker (MSW) to deliver compassionate, patient‑centered support in patients’ homes across Prince George’s County. You’ll assess psychosocial needs, connect families with community resources, support care planning and advocate for goals that promote dignity and independence.

We value autonomy, flexible scheduling, and meaningful 1:1 patient relationships.

Qualifications

• Master's Degree from a school of social work accredited by the CSWE.
• Maryland Social Work License required.
• Minimum 2 years of social work experience in a health care setting, hospice preferred.

Responsibilities

• Assess psychosocial needs of patients and families.
• Connect patients with community resources and support services.
• Support care planning and advocate for patient goals.
• Provide 1:1 patient relationships with autonomy in home settings.
