Job summary

Residential Home Health and Hospice is seeking a Hospice Medical Social Worker to provide patient- and family-centered psychosocial care in patients' homes around Lisle, IL. You will work with an interdisciplinary team to support emotional well-being and end-of-life planning.

The ideal candidate holds a Master’s in Social Work, an active MSW license, and can drive to patient locations. This is a full-time, 40-hour-per-week role with comprehensive benefits and growth opportunities.

Qualifications

• Master’s in Social Work from graduate school.
• Must hold, in good standing, a license as a Medical Social Worker.
• Drivers license and ability to travel to patient homes.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Conduct psychosocial and bereavement assessments and develop care plans.
• Provide counseling and connect patients with resources for end-of-life goals.
• Apply critical thinking and ensure accurate EMR documentation.
• Travel to patient homes within the assigned territory.
