Job summary

Residential Home Health and Hospice in Lisle, IL, is seeking a Medical Social Worker (MSW) for our hospice program. This role focuses on delivering compassionate, patient-centered care within a high-quality interdisciplinary team in patients’ homes across Lisle and surrounding areas.

The MSW conducts psychosocial and bereavement assessments, develops care plans, and provides counseling and resource connections to patients and families.

Qualifications

• Master’s in Social Work from a CSWE-accredited program.
• 1–2 years post-graduate social work experience in a health care setting.
• Licensed Medical Social Worker in the practicing state.
• Valid driver’s license and ability to drive to patient locations ~20% of day.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Conduct psychosocial and bereavement assessments and develop care plans.
• Provide counseling and connect patients and families with resources.
• Apply critical thinking to address complex situations and ensure compliant EMR documentation.

• Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation

• Work independently but never alone

MSW Qualifications

• Master’s in Social Work from graduate school accredited by the Council of Social Work

• One (1) to two (2) years post graduate social work experience in a health care setting

• Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
