Residential Home Health and Hospice in Northbrook, IL is seeking a Hospice Medical Social Worker (MSW) to deliver compassionate, patient-centered care at patients’ homes within the Northbrook and surrounding areas. You will work with an interdisciplinary team to support comfort, dignity, and quality of life.

The role requires an MSW with licensure, 1–2 years in healthcare, and ability to drive ~20% of the time. This full-time position offers leadership, growth, and a strong benefit package.

Formación

• Master's in Social Work from graduate school accredited by the Council of Social Work
• One to two years post graduate social work experience in a health care setting
• Must hold, in good standing, a license as a Medical Social Worker issued by the state
• Current driver's license and ability to spend ~20% of your day driving to/from patient locations

Responsabilidades

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team
• Conduct psychosocial and bereavement assessments and develop care plans
• Provide counseling and guidance to patients and families and connect resources
• Ensure accurate, timely EMR documentation and advocate for patient wishes
• Travel to patient homes within assigned territory
