Job summary

Residential Home Health and Residential Hospice is seeking a Hospice Medical Social Worker (MSW) for the Chicago, IL area. This full-time position involves providing in-home hospice social work services, collaborating with an interdisciplinary team to support comfort and dignity at end of life.

The MSW will perform psychosocial assessments, offer counseling, coordinate resources, and maintain compliant EMR documentation. Travel to patient homes is required.

Qualifications

• Master's in Social Work from graduate program
• 1–2 years post-graduate social work experience in healthcare
• Valid Medical Social Worker license in the state of work
• Valid driver's license and ability to travel ~20% of time

Responsibilities

• Deliver compassionate, patient-centered hospice social work
• Conduct psychosocial and bereavement assessments and develop care plans
• Provide counseling and connect patients/families with resources
• Ensure accurate, timely EMR documentation
• Work independently while collaborating with interdisciplinary team
• Travel to patient homes within assigned territory
