Job summary

American Medical Hospice Care is seeking a dedicated Hospice Medical Social Worker (MSW) to provide emotional support, counseling, and resource coordination for hospice patients and families. You will work collaboratively with the clinical team to address psychosocial and practical needs, helping maintain quality of life and autonomy throughout the care journey.

Qualifications

• MSW degree from an accredited school.
• Licensed in the state of Texas.
• Minimum of one year experience in medical, palliative, or hospice social work preferred.
• Strong interpersonal skills with a compassionate and non-judgmental approach.
• Valid driver’s license and reliable transportation; travel to patient homes required.
• Knowledge of Medicare/Medicaid hospice regulations a plus.

Responsibilities

• Conduct psychosocial assessments of patients and families upon admission and throughout care.
• Develop individualized care plans addressing emotional, social, financial, and practical needs.
• Provide crisis intervention, grief counseling, and support for anticipatory loss.
• Assist patients and families with end-of-life planning, including advance directives and funeral arrangements.
• Advocate for patients and connect families with community resources and support services.
• Participate in interdisciplinary team meetings and coordinate care with nurses, chaplains, and physicians.
• Maintain accurate and timely documentation in compliance with regulatory and organizational standards.
