Job summary

Anchor Health in California seeks a Medical Social Worker (MSW) to support patients and families facing terminal illness. The MSW will perform psychosocial assessments, coordinate care plans, and liaise with community agencies within an interdisciplinary hospice team.

Requirements include an MSW accredited by CSWE, CPR certification, and a valid driver's license. Hospice experience and strong communication skills are preferred; travel within the service area may be required.

Qualifications

• Graduate MSW accredited by CSWE.
• One year social work experience in healthcare; hospice preferred.
• Licensed driver with insured vehicle.
• Current CPR certification.
• Excellent verbal and written communication; well organized.

Responsibilities

• Assess the psychosocial status of patients and families, communicating findings to the nurse and team.
• Perform social evaluations, address caregiver needs, provide crisis intervention when needed.
• Educate interdisciplinary team on cultural needs, communication, family roles and taboos.
• Maintain clinical records for patients referred to social work.
• Assist in preparing advanced directives and coordinating referrals.
• Provide information to patients, families, and community agencies; liaison with external agencies.
• Participate in interdisciplinary meetings and care planning; support psychosocial aspects of care.
• Assist with funeral planning, financial, legal, and health-care decision responsibilities.
