Job summary

Concho Hearts Hospice is seeking an experienced Hospice Account Executive / Community Liaison to join our Beaumont team. This role focuses on building referral partnerships, educating providers about hospice services, and helping patients and families access compassionate end-of-life care.

The liaison will develop relationships with hospitals, physicians, and care facilities, educate community partners, and coordinate admissions with the clinical team to ensure smooth patient experiences.

Qualifications

• Hospice marketing experience preferred.
• Strong ability to build and maintain referral networks.
• Excellent organizational and time-management skills.
• Comfortable presenting hospice education to diverse groups.
• Ability to collaborate with clinical teams for admissions.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, assisted living communities, and SNFs
• Grow referral sources throughout the service area
• Provide hospice education to healthcare providers and community partners
• Meet with patients and families to explain hospice services and obtain consents
• Coordinate closely with the clinical team to ensure seamless admissions
• Represent Concho Hearts Hospice as a trusted community resource
