Job summary

Concho Hearts Hospice is seeking an experienced Hospice Marketer / Community Liaison to join our growing Fort Worth team. You will build referral partnerships, educate providers about hospice services, and help patients and families access compassionate end‑of‑life care.

Ideal candidates are relationship‑driven, self‑motivated, and passionate about hospice care, with the ability to work closely with clinicians to coordinate admissions and patient transitions.

Qualifications

• Minimum 2 years of hospice marketing experience.
• Ability to build referral partnerships and educate providers.
• Comfortable presenting hospice education to groups and providers.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, and care facilities.
• Grow referral sources throughout the Fort Worth service area.
• Provide hospice education to healthcare providers and community partners.
• Meet with patients and families to explain hospice services and obtain consents.
• Coordinate with the clinical team to ensure smooth admissions.
• Represent Concho Hearts Hospice as a trusted community resource.
