PIH Health is seeking a Licensed Vocational Nurse (LVN) to serve as Intake Coordinator for Hospice services. Under the supervision of the Clinical Director, you will coordinate intake information, schedule services, and ensure smooth flow of clinical data to support patient outcomes.

The role requires at least one year of LVN experience (home health or hospice preferred), current CPR, and a California driver’s license.

Competenze

• Minimum one year experience as LVN; Home Health or Hospice experience preferred
• Current CPR card
• Current California driver's license and auto insurance preferred
