Job summary

Alternate Solutions Health Network seeks a dedicated Hospice LPN to provide skilled nursing at patients' homes through Mercy Medical Center Home Health & Hospice. You will collaborate with the RN and interdisciplinary team to promote comfort, safety and quality of life for hospice patients.

Current Ohio LPN license required; one year hospice experience preferred. You’ll educate families, manage medications and maintain compliant clinical documentation in a patient-centered environment.

Qualifications

• Current LPN license in the state of employment.
• One year of LPN experience strongly preferred; hospice experience preferred.
• Strong communication and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently, prioritize tasks, and make appropriate nursing judgments.
• Ability to work effectively as part of an interdisciplinary team.

Responsibilities

• Provide skilled nursing care according to the patient’s plan of care and physician orders.
• Monitor patient condition and report changes to the RN.
• Administer and reconcile medications as prescribed.
• Educate and support patients and families regarding disease progression and comfort care.
• Complete accurate clinical documentation in accordance with agency and regulatory requirements.
• Communicate with the care team regarding patient needs and changes.
• Participate in on-call responsibilities, case conferences, required training, and meetings.
• Follow all agency policies, procedures, and applicable regulations.

Mercy Medical Center Home Health and Hospice Schedule: Tuesday-Friday 10:00PM - 8:00AM

How you will make a difference!! As a Hospice LPN with Mercy Medical Center Home Health & Hospice, you will provide compassionate, skilled nursing care to patients in their homes while supporting patients and families throughout their hospice journey. EDUCATION & CREDENTIALS

• Graduate of an approved practical nursing program.
• Current, unrestricted LPN license.
