Job summary

Alternate Solutions Home Health, LLC is seeking an LPN to provide home-based nursing care under physician orders, collaborating with an interdisciplinary team. You’ll work autonomously, manage your schedule efficiently, and adhere to state practice guidelines while supporting patients and families during recovery.

Qualifications

• LPN license current and in-state.
• Minimum one year of LPN experience in an acute care setting; home care preferred.
• Valid driver’s license and auto insurance in your name.
• Capable of all physical demands.

Responsibilities

• Plan of Care: provide skilled nursing care and monitor patient conditions.
• Minimize Patient Risk: administer meds and educate patients to reduce rehospitalizations.
• Family Educator/Advocate: educate patient and family using teach-back methods.
• Policies: complete clinical documentation per protocol and Medicare guidelines.
• Rules and Regulations: follow agency policies and communicate schedule changes.
• Operations: participate in in-service trainings and meetings.

You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager. HOW YOU'LL WORK

You’ll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You’ll be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY

• Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition.
• Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures.
• Family Educator/Advocate: Educate the patient and family on the disease process using teach‑back methods to ensure understanding.
• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate schedule changes to schedulers or supervisors.
• Operations: Attend in‑service training and mandatory agency meetings.

REQUIREMENTS

• Licensed Practical Nurse (LPN) with current license in the state of employment
• Minimum one‑year experience as an LPN in an acute care setting. Home care experience preferred.
• Valid driver’s license and auto insurance in your name as a driver
• Capable of all physical demands
