Job summary

Redcrest Hospice LLC is seeking a Registered Nurse (RN) I - Hospice to provide and plan professional nursing care under physician direction. You will deliver direct patient care, assist with examinations, document patient conditions, vital signs, diagnoses, and treatments, and administer medications as directed to support comfort and symptom control for hospice patients.

The role requires 0–2 years of experience, valid RN licensure in relevant states, and a Bachelor’s degree or higher in nursing

Monday - Friday 8am-4:30pm, back-up on call rotation, holiday on call rotation

SUMMARY A Registered Nurse (RN) I - Hospice

A Registered Nurse (RN) I - Hospice provides and plans professional nursing care to patients under the direction of a physician. This role involves delivering direct patient care, assisting with examinations, accurately documenting patient conditions, vital signs, diagnoses, and prescribed treatments. The Level 1 RN also administers treatments and medications as directed, contributing to the comfort and symptom control of hospice patients.

KEY RESPONSIBILITIES
  • Provides skilled nursing care in a patient’s home as ordered by the attending physician
  • Observes and monitors patient conditions
  • Performs assessments to develop an individualized plan of care and adjusts as needs change
  • Administers medication as prescribed by the physician
  • Manages multi-disciplinary care as applicable
  • Promotes continuity of care with appropriate admissions, transfers and discharges
  • Counsels’ patient and family on the disease/injury processes and how to manage
  • Incorporates patient and family in development of plan of care
  • Listens to patient and family members
  • Oversees and supervises total care of patient provided by Aides and LPNs
  • Evaluates the treatments and medical condition of the patient according to the plan of care
  • Other duties as assigned
QUALIFICATIONS
  • Performs work under minimal supervision. Handles complex issues and problems and refers only the most complex issues to higher-level staff. Possesses comprehensive knowledge of subject matter.

EDUCATION AND CREDENTIALS

Valid RN license in the state(s) where service is provided

Bachelor's degree in nursing or healthcare-related field preferred
