Job summary

Mercy Hospice is seeking a Licensed Practical Nurse I (LPN) to provide comprehensive skilled nursing care in patients’ homes as ordered by the physician and medical director. You will educate patients and families, ensure patient safety, and maintain complete documentation following Medicare guidelines.

The role requires clinical judgment, strong communication with families and the care team, and participation in on-call duties and team meetings. Prior hospice experience is preferred.

Qualifications

• One year of LPN experience in an acute care setting is strongly recommended.
• Hospice care experience is desired.
• Ability to create positive impressions and communicate with a variety of people.

SUMMARY

The Licensed Practical Nurse I (LPN), Hospice provides comprehensive skilled care as ordered by the attending physician/medical director by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations, as outlined by the state.

KEY RESPONSIBILITIES

• Provides skilled nursing care in a patient’s home as ordered by the attending physician/medical director
• Performs care by following order sets and goals made by RN
• Observes and monitors patient conditions
• Notifies RNCM of any changes in patient condition
• Reconciles medications prescribed by the physician
• Counsels patient and family on the disease/progression and how to manage
• Incorporates patient and family in development of plan of care
• Listens to patient and family members
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Documents all aspects of treatment, assessments, and patient education
• Communicates with scheduler any changes outside the normal
• Participates in all on-call requirements and case conferences
• Attends in-service trainings and mandatory agency meetings
• Maintains a professional appearance as a representative of the company
• Understands and practices agency policies and procedures
• Other duties as assigned

QUALIFICATIONS

• One year of experience as an LPN in an acute care setting is strongly recommended. Hospice care experience is desired.

EDUCATION AND CREDENTIALS

Licensed Practical Nurse with current license in the state of employment.
