Responsibilities

  • Provide skilled nursing care in patients' homes as directed.
  • Follow order sets and goals created by RN and MD.
  • Educate patients and families about disease progression and care plans.
  • Document treatment, assessments and education per agency and Medicare rules.
  • Coordinate with scheduler for changes and participate in on-call duties.

We CARE for our patients like they are our own FAMILY.Schedule: Monday - Friday, 8:15AM - 5:00PM with on-call requirementsAgency: Mercy HospiceSUMMARYThe Licensed Practical Nurse I (LPN), Hospice provides comprehensive skilled care as ordered by the attending physician/medical director by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations, as outlined by the state.KEY RESPONSIBILITIESProvides skilled nursing care in a patient’s home as ordered by the attending physician/medical directorPerforms care by following order sets and goals made by RNObserves and monitors patient conditionsNotifies RNCM of any changes in patient conditionReconciles medications prescribed by the physicianCounsels patient and family on the disease/progression and how to manageIncorporates patient and family in development of plan of careListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of treatment, assessments, and patient educationCommunicates with scheduler any changes outside the normalParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresOther duties as assignedQUALIFICATIONSOne year of experience as an LPN in an acute care setting is strongly recommended.
