Job summary

Alternate Solutions Health Network seeks a Licensed Practical Nurse (LPN) to provide skilled home-based care. You will administer medications, monitor patient conditions, and collaborate with physicians, families, and co-workers to develop and follow care plans.

Ideal candidates have 1+ year in acute care, home health experience preferred, and possess a valid LPN license. Travel to patient locations is required; the role supports a comprehensive benefits package and on-call responsibilities.

Responsibilities

QUALIFICATIONS & ATTRIBUTES:

• Licensed Practical Nurse with current license in the state of employment.
• Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
• Home care experience is desired.

MAJOR AREAS OF RESPONSIBILITY:

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care by following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on the disease/injury processes and how to manage.
• Incorporates patient and family in development of plan of care.
• Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
• Listens to patient and family members.
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
• Documents all aspects of treatment, assessments, and patient education.
• Maintains active LPN license in state of employment.
• Communicates with scheduler any changes outside the normal.
• Participates in all on-call requirements and case conferences.
• Attends in-service trainings and mandatory agency meetings.
• Maintains a professional appearance as a representative of the company.
• Understands and practices agency policies and procedures.
• Other duties as assigned.

ESSENTIAL FUNCTIONS:

• Drives to patient’s primary location per scheduled visit.
• Daily attendance at assigned visit locations.
• Documents all aspects of subsequent visits within 24 hours of visit to meet Medicare guidelines.
• Completes and submits all required documentation within specified company requirements.
• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse.
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
