Job summary
Beth Israel Lahey Health is seeking a Hospice LPN to join our team serving Boston, Reading, and North Shore locations. You will perform nursing visits, provide hospice care under the RN Case Manager, and collaborate with physicians to adjust care plans.
This role requires MA LPN licensure, BLS, and 2+ years of relevant experience, with hospice experience preferred. You will participate in weekend/holiday rotations, document care, and contribute to interdisciplinary case conferences.
Qualifications
- Graduate from an NLNAC-accredited LPN program.
- Current Massachusetts LPN licensure and BLS certification.
- Minimum of two years of LPN experience in acute care, rehab, or home care; hospice experience preferred.
Responsibilities
- Perform nursing visits to patients as delegated by the Hospice RN Case Manager.
- Provide hospice nursing care utilizing palliative care principles to meet the plan of care.
- Observe and report significant changes to the MD and RN Case Manager.
- Communicate with physicians regarding patient status and obtain orders as required.
- Participate in weekend/holiday rotation and bi-weekly hospice case conferences.
- Document services in problem-oriented records and care plan updates.
