Job summary

Beth Israel Lahey Health is seeking an experienced Licensed Practical Nurse for the Amesbury/Salisbury/West Newbury/Newbury territory to deliver professional nursing care under the supervision of an RN Case Manager. The LPN will perform visits, provide hospice care, monitor patient status, and educate families while respecting cultural beliefs and preferences.

The role requires a MA LPN license, NLNAC-accredited training, BLS, and 2+ years in acute, rehab, or home care with hospice experience

Qualifications

Active Massachusetts LPN license.
Graduate from NLNAC-accredited LPN program.
American Heart Association BLS certification.
2+ years of LPN experience in acute, rehab, or home care; hospice preferred.

Responsibilities

Perform nursing visits as assigned by the RN Case Manager/Clinical Manager.
Provide hospice nursing care using current principles for symptom management as directed by the RN.
Observe and report significant changes in patient health status to the MD and RN Case Manager.
Participate in health teaching for patients and families.
Respect patients’ cultural beliefs and treatment preferences.
Communicate effectively with patients, caregivers, and the interdisciplinary healthcare team.
Attend regular IDT meetings.
Maintain confidentiality per HIPAA and agency policies.
