Job summary

Beth Israel Lahey Health at Home is seeking a dedicated Hospice LPN Team Resource in Watertown, MA. You will perform hospice nursing visits, contribute to assessment and care planning, and collaborate with the RN Case Manager to ensure high-quality patient care in a home setting.

You will hold an active MA LPN license, maintain BLS, and bring 2+ years of LPN experience in acute, rehab or home care. Hospice experience is preferred; weekend/holiday rotations are included.

Qualifications

• Graduate from an NLNAC-accredited LPN program.
• Current Massachusetts LPN licensure.
• Two years of LPN experience in acute care, rehab or home care; hospice experience preferred.

Responsibilities

• Perform hospice nursing visits to patients as delegated by the Hospice RN Case Manager/Clinical Manager.
• Provide hospice nursing care focusing on palliative care and symptom management.
• Observe and report significant changes to the physician and RN Case Manager; communicate changes promptly.
• Document services using problem-oriented records and participate in hospice case conferences; support weekend/holiday rotations.
