Job summary

Ohio’s Hospice in Dayton, OH is seeking a Hospice LPN for night shift (8pm-8am) to provide 1:1 bedside care to patients at home or in facilities within the Dayton/Springfield area. This role requires driving between assignments and delivering compassionate, patient-centered care under an RN supervision.

Qualified candidates must be an Ohio-licensed LPN, CPR certified, with a valid driver's license and clean driving record.

Qualifications

• Licensed LPN in Ohio with no current board actions.
• One year of nursing experience preferred.
• CPR certification.
• Ability to drive to Dayton/Springfield area and document care electronically.
• Must pass criminal background check and a 10-panel drug screen.

Responsibilities

• Provide 1:1 bedside care for patients actively declining.
• Care may occur in patients' homes or facilities under RN supervision.
• Drive between assignments in Dayton/Springfield area.
• Document services accurately in the electronic records system.

Hospice Individualized Care LPN - Making every moment matter

• This is a PT night shift position 8pm-8am and will require DRIVING to patients within the Dayton/Springfield area
• This position will provide 1:1 with the patient at the bedside for patients that are actively declining

As a Hospice Individualized Care LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.

Qualifications for the Hospice LPN:

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR Certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
• Must be able to pass a 10 panel drug screen
