Job summary

Ohio’s Hospice is seeking an Individualized Hospice Care LPN for full-time day shifts in Dayton, OH. You will deliver compassionate 1:1 bedside care to patients in homes or facilities under RN supervision, with driving to cases across Dayton and nearby counties.

This role requires an Ohio LPN license, CPR, ability to drive in varying conditions, and a clean background/drug screen. A competitive package and benefits are offered to support patient-focused care.

Qualifications

• Registered LPN with the State of Ohio, no board actions.
• 1 year nursing experience preferred.
• CPR certification.
• Computer skills to document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Pass a criminal background check.
• Pass a 10 panel drug screen.

Responsibilities

• Provide care for patients in homes or facilities under RN supervision.

Individualized Hospice Care LPN - Making moments matter

As a Hospice LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.

This is a FT day shift 8am-8pm and will require DRIVING to patients within the Dayton and surrounding county areas. You will provide 1:1 with the patient at the bedside for patients that are actively declining

Qualifications for the Hospice Individualized Care LPN:

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
• CPR Certified.
• Computer skills sufficient to properly document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid Driver's License with Safe Driving Record.
• State Minimum Automobile Insurance Coverage.
• Must be able to pass a criminal background check
