Job summary

Ohio’s Hospice, Inc. is seeking an Individualized Care LPN for full-time day shift in Dayton, OH. You will provide 1:1 bedside hospice care in patients’ homes or facilities under the direction of an RN.

The role requires driving to patients within Dayton and surrounding counties, with a day shift from 8am to 8pm. Qualifications include an Ohio LPN license, CPR certification, and the ability to document care professionally.

Qualifications

• Licensed Practical Nurse (LPN) registered with the State of Ohio with no board actions.
• One year of nursing experience preferred.
• CPR certification.
• Computer skills sufficient to document services and care.
• Ability to drive during daytime, nighttime or inclement weather.
• Valid driver’s license with a safe driving record.
• State minimum automobile insurance coverage.
• Must pass a criminal background check.
• Must pass a 10-panel drug screen.

Responsibilities

• Provide 1:1 bedside nursing care to patients at home or in facilities.
• Drive to patients within Dayton and surrounding counties.
• Deliver compassionate hospice care under RN supervision.

Individualized Hospice Care LPN - Making moments matter

As a Hospice LPN you will provide care for patients in families in homes/facilities, under the direction and supervision of an RN.

This is a FT day shift 8am-8pm and will require DRIVING to patients within the Dayton and surrounding county areas. You will provide 1:1 with the patient at the bedside for patients that are actively declining

Qualifications for the Hospice Individualized Care LPN:

• Registered LPN with the State of Ohio without any board actions.
• One year of nursing experience preferred.
