Qualifications

• Current LPN license in the State of Ohio in good standing.
• CPR certification required.
• Basic computer proficiency for timely and accurate clinical documentation.

Responsibilities

• Deliver skilled nursing services to patients in homes or care facilities under RN supervision; follow plan of care and document care provided.
• Serve as a key member of the hospice care team to ensure respectful, individualized end-of-life care.
