Job summary

Providence at Home with Compassus is seeking an LPN/LVN to deliver hospice nursing care under the supervision of a registered nurse. The role focuses on symptom management, family education, and ensuring continuity of care in the home setting.

The ideal candidate will have an active LVN license in CA, OR, or WA and at least 1 year of hospice or home health experience, with strong communication and documentation skills. Knowledge of Medicare guidelines is preferred.

Qualifications

• Minimum 1 year as an LVN/LPN in hospice or home health preferred.
• Knowledge of Medicare Conditions of Participation and other regulatory guidelines.
• Ability to educate patients and families about care plans and changes in condition.

Responsibilities

• Provide LPN/LVN-level hospice nursing services under RN supervision.
• Observe and report patient data to the RN for Plan of Care development.
• Implement nursing interventions per the Plan of Care and hospice policies.
• Educate patients and families about care needs and anticipated changes.
• Document observations and communications accurately.
• Triage and medication desk duties as assigned.
