Qualifications

• Current LPN license in Washington state.
• Active BLS certification required.
• Hospice, home health, or palliative care experience preferred.

Responsibilities

• Provide skilled nursing care to hospice patients per orders and policies.
• Administer medications, monitor conditions, and document care.
• Educate patients and families on care plans and symptom management.
• Collaborate with RNs, social workers, and aides to support goals.
• Respond to patient and family needs, including urgent care.
• Maintain compliance with state regs and Medicare guidelines.
• Participate in weekend on-call rotation.
