Job summary

Providence at Home with Compassus is seeking an LPN/LVN to provide holistic hospice nursing under RN supervision in home-based care settings. You will follow the Plan of Care and support patients and families at the end of life.

You will observe patient data, implement interventions, educate families, and coordinate care with the hospice team. Knowledge of Medicare Conditions of Participation and EHR systems is valued.

Qualifications

• Nursing education/licensing level must meet state requirements.
• Experience in hospice or home health is preferred.
• Ability to educate patients and families about care needs and plans.

Responsibilities

• Provide LPN/LVN hospice nursing services as supervised by RN.
• Observe and report patient data for Plan of Care.
• Implement nursing interventions per Plan of Care and policies.
• Anticipate disease progression and manage symptoms.
• Educate patients and families about care needs and changes.
• Assist with understanding insurance benefits and cost-effective care.
• Document observations and communications accurately.
• Manage caseload and update RN on Plan of Care.
