Job summary

Providence at Home with Compassus seeks an LPN/LVN to provide holistic hospice nursing care under the supervision of a Registered Nurse. You will support patients and families by anticipating changes, promoting comfort, ensuring continuity of care, and enhancing quality of life in end-of-life settings.

Responsibilities include implementing the Plan of Care, observing patient data, educating families, documenting accurately, and collaborating with the hospice team to deliver compassionate care.

Qualifications

• Must hold California LVN license upon hire.
• 1 year LVN/home care experience preferred.
• Infusion and venipuncture certification required.
• National Provider BLS (American Heart Association) within 30 days of hire.

Responsibilities

• Provide hospice nursing services as established by supervising RN.
• Observe and report patient data to the RN for Plan of Care.
• Implement interventions per Plan of Care and hospice policies.
• Educate patients and families about care needs and changes.
• Document observations and communications accurately and timely.
• Manage caseload and communicate Plan of Care updates to the RN.
• Collaborate with the hospice team; apply critical thinking to issues.
• Adhere to hospice policies and Conditions of Participation.
