Blue Summit Hospice and Palliative Care in Georgia seeks a Hospice LPN DON Assistant to support the Director of Nursing in delivering compassionate end-of-life care. The role focuses on clinical oversight, staff coordination, and ensuring regulatory compliance to maintain high-quality patient services.

You will contribute to staff education, documentation accuracy, and quality improvement initiatives, helping to ensure smooth nursing operations and excellent patient comfort, dignity, and family

Qualifications

• LPN license in good standing in state of practice.
• Minimum of 2 years of clinical nursing experience.
• Minimum 2 years experience scheduling in Homecare Homebase.
• Basic knowledge of hospice care principles, symptom management, and end-of-life care.
• Familiarity with healthcare regulations and documentation standards.
• Strong communication and interpersonal skills with patients, families, and healthcare teams.

Responsabilités

• Assist the Director of Nursing in managing daily nursing operations and staff coordination within the hospice care team.
• Provide direct patient care including medication administration, symptom management, and monitoring patient conditions.
• Ensure compliance with hospice policies, state and federal regulations, and accreditation standards through regular audits and documentation review.
• Support nursing staff by providing guidance, training, and assistance with clinical procedures and documentation.
• Facilitate communication between patients, families, interdisciplinary team members, and external healthcare providers to promote coordinated care.
• Participate in quality assurance and performance improvement activities to enhance patient care outcomes.
• Maintain accurate and timely clinical records, including patient assessments, care plans, and progress notes.
