Qualifications

LPN license in good standing in GA.
Minimum 2 years of clinical nursing experience, preferably in hospice or palliative care.
Experience scheduling in Homecare Homebase.
Familiarity with hospice care regulations and documentation standards.
Strong communication with patients, families, and healthcare teams.

Responsibilities

Assist the Director of Nursing in managing daily nursing operations and staff coordination.
Provide direct patient care including medication administration and symptom management.
Ensure compliance with hospice policies and regulatory standards through audits and reviews.
Support nursing staff with guidance, training, and documentation assistance.
Facilitate communication between patients, families, and interdisciplinary teams.
Participate in quality assurance and performance improvement activities.
Maintain accurate clinical records with assessments, care plans, and progress notes.
