Job summary

Blue Summit Hospice and Palliative Care is seeking a Hospice LPN DON Assistant who will support the Director of Nursing in delivering compassionate, high-quality end-of-life care. The role emphasizes clinical oversight, regulatory compliance, and effective communication across nursing staff, patients, and families.

Responsibilities include direct patient care, staff education, and documentation accuracy to ensure smooth nursing operations.

Qualifications

• LPN license in good standing in the state of practice.
• Minimum 2 years of clinical nursing experience, hospice preferred.
• Minimum 2 years scheduling experience using Homecare Homebase.
• Familiarity with hospice care principles, regulations, and documentation standards.

Responsibilities

• Assist the Director of Nursing in managing daily nursing operations and staff coordination within the hospice team.
• Provide direct patient care including medication administration and symptom management.
• Ensure compliance with hospice policies and state/federal regulations through audits and documentation review.
• Support nursing staff with guidance, training, and procedural assistance.
• Facilitate communication between patients, families, and interdisciplinary teams to coordinate care.
• Participate in quality improvement activities to enhance patient outcomes.
• Maintain accurate clinical records, including assessments, care plans, and progress notes.
