Job summary

Guardian Angel Hospice is seeking a compassionate LPN to provide specialized nursing care within the hospice program. The LPN will implement the care plan, assist RNs and physicians, and educate patients and families while upholding confidentiality.

Responsibilities include documenting care, prepping equipment, and supporting the IDT in end-of-life care. Strong judgment, organization, and reliable transportation are essential to succeed in this role.

Qualifications

• Must hold an LPN license.
• Reliable transportation and a valid driver’s license.
• Ability to make sound nursing judgments in diverse field settings.
• Experience in hospice or end-of-life care preferred.

Responsibilities

• Deliver nursing services per the individualized care plan created by the physician and IDT.
• Prepare clinical and progress notes detailing services provided.
• Assist the RN and/or physician with end-of-life care tasks.
• Support the RN in implementing the plan of care.
• Prepare equipment and materials for treatment while following aseptic techniques.
• Help patients learn appropriate self-care techniques.
• Assess patient and family responses to care and communicate with team.
• Submit documentation per agency policies and participate in IDT meetings.
