Job summary

Guardian Angel Hospice in Kokomo seeks a compassionate LPN to deliver specialized nursing services based on the physician's care plan and the IDT. You will support and educate patients and families while maintaining confidentiality.

Qualifications

• Provide nursing services based on the care plan created by physicians and the IDT.
• Assist RN/physician with end-of-life care.
• Prepare equipment and materials for treatment using aseptic technique.
• Communicate care plans and progress to patients and families.

Responsibilities

• Deliver services per organizational policies and scope of practice.
• Prepare clinical and progress notes.
• Assist with specialized duties related to end-of-life care.
• Support the RN in implementing the plan of care.
• Prepare equipment and materials for treatment while adhering to aseptic techniques.
• Attend in-service programs and interdisciplinary team meetings.
